Request for a Medical Report

At Oriental Melaka Straits Medical Centre, our Medical RecordsDepartment (MRD) provides service for the request of: Medical Record | Medical Report

According to the Guideline of The Malaysian Medical Council, a medical record is documented information about the health of an identifiable individual recorded by a practitioner or other healthcare professional. It contains sufficient information to identify the patient, support the diagnosis based on history, physical examination, and investigation, justify the professional management given, record the course and result thereof and ensure the continuity of care provided by practitioners and other healthcare workers. 

While medical reports are documents prepared by a practitioner on a patient based on medical records. Opinion by an expert may also be part of a medical report.

Availability of medical report – medical report will be processed within 14 working days after receiving a complete request and payment, excluding Sundays and public holidays, the medical report or insurance claim will be available for collection. However, there may be delays in cases where the doctor is on leave or out of the country. Our staff will inform and update applicants through email and phone call.
